Terms of Service

Welcome to Rigby Lake Memory Care. By enrolling a resident in our community, touring our facility, or using our services, you agree to the following Terms of Service. These terms are designed to ensure clarity, safety, and mutual respect for residents, families, and staff.


1. Services Provided

Rigby Lake Memory Care provides residential care services specifically for seniors with Alzheimer’s, dementia, and memory care needs. Services include:

  • Room and board (private or semi-private)
  • 24/7 staff support
  • Medication management and care plans
  • Daily meals and snacks
  • Housekeeping, laundry, and maintenance
  • Social and recreational activities
  • Access to in-house medical providers, nursing support, physical therapy, and partnerships with home health and hospice


2. Admission & Fees

  • A non-refundable Admission Fee of $850 is required to reserve a room for up to 10 days.
  • A one-time, non-refundable Community Fee of $1,250 is due upon admission.
  • Monthly room rates and care level fees apply based on the resident’s needs.
  • Prices are subject to change with advance written notice.
  • Payment may be made via Medicaid, VA benefits, long-term care insurance, or private pay.


3. Resident Care

  • Care plans are customized to each resident’s needs and may be updated as conditions change.
  • Families are encouraged to participate in care plan meetings.
  • Rigby Lake Memory Care is not liable for conditions outside our control, including natural decline due to age or illness.


4. Family Responsibilities

  • Families agree to provide accurate medical and personal information upon admission.
  • Any outside medical providers must be coordinated with facility staff.
  • Families remain financially responsible for all fees and charges not covered by Medicaid, insurance, or VA benefits.


5. Resident Rights

Residents have the right to:

  • Respectful, dignified care at all times
  • Participate in activities and community life
  • Privacy in their living space
  • Communication with family, friends, and providers


6. Limitation of Liability

While we take every measure to provide a safe, secure, and caring environment, Rigby Lake Memory Care is not responsible for:

  • Injuries or medical issues that occur due to pre-existing conditions or natural aging processes
  • Loss or damage to personal belongings not secured by staff
  • Events outside of reasonable staff control


7. Termination of Services

  • Services may be terminated by either party with written notice.
  • Rigby Lake Memory Care may require relocation if a resident’s needs exceed the level of care provided at our facility.


8. Amendments

We may update these Terms of Service from time to time. Families will be notified of material changes in writing.
